dc.description.abstractThis chapter discusses the principles of some important skills for working in the community: the home visit, referral, community resources and case management. Home visiting is an integral component of many community nursing roles, particularly domiciliary nursing, hospital in the home nursing, aged care assessment teams, and hospital outreach/follow-up programs. Home visiting is also important to other community nursing roles such as child health nursing, practice nursing and dialysis nursing. While it can be argued that it is easier and more efficient to ask patients or clients to visit a hospital, clinic or health centre, there may be important reasons for community nurses to make home visits. An important aspect of home visiting is managing time and resources to achieve the best outcomes. Prior to attending a home visit, it is important to plan. Development of home visiting practice is developing, and new approaches to practice will occur as further research into home visiting practices is carried out.en_US
