    Abstract
    CREATE Foundation recently published research into the transitioning-from-care process in Australia (McDowall, 2009). Based on the findings of this Report Card, it was decided to develop a social marketing program ("What's the Plan?") to focus on the failure of the current system to assist young people leaving care in planning for their future. This paper presents a rationale for the design of the program along with discussion of the research on which it is based. Finally, the interventions invoked will be described.
