A 55‐year‐old woman, who has survived 5 years since undergoing left hepatectomy and extrahepatic bile duct resection for hilar cholangiocarcinoma, present with jaundice and mild ascites. Computed tomography imaging revealed portal vein stenosis and moderate biliary tree dilatation with a 2‐ to 3‐cm mass at the junction of the anterior and posterior ducts and portal branches near the roux loop reconstruction (Fig. 1a). Her bilirubin level was 89 mg/dL. She underwent FOLFIRINOX chemotherapy but did not achieve any biochemical or imaging response.
