Breast implant anaplastic large cell lymphoma (BIA‐ALCL) is linked to textured implants and frequently presents with a spontaneous peri‐implant seroma that develops at least 12 months following implantation.1 Delayed seromas are only seen in 0·05–0·1% of patients and amongst these an estimated 10% will have BIA‐ALCL.2 A fine needle aspirate (FNA) sent for flow cytometry and cytology are the optimal investigations to analyse a peri‐implant seroma as most cases of BIA‐ALCL are limited to the peri‐implant fluid. Expression of CD30 is a fundamental finding in BIA‐ALCL; however, it is important to note CD30 expression is non‐specific and can be expressed by benign or inflammatory cells. There is very little in the literature outlining the typical flow cytometric appearance of BIA‐ALCL with fewer than 10 cases reported to date.3-5 We undertook an evaluation of the flow cytometry characteristics of patients with breast implants undergoing FNA for a seroma, and specifically focused on those patients with confirmed BIA‐ALCL.
