Abstract
Aims: This study aimed to investigate the feasibility of deriving a frailty index (FI) and explore the understand-ing and acceptability of frailty assessment among patients presenting to a kidney transplant assessment clinic.Methods: A mixed methods study was conducted. A questionnaire was completed with patients presenting to a kidney transplant assessment clinic (N = 91) which in-cluded 58 variables representing multiple health domains. Deficits were summed and divided by total number to de-rive a FI. Semi structured interviews were conducted on a sub- sample (N = 29) to explore acceptability of frailty assessment and views on frailty. Qualitative data was ana-lysed using reflexive thematic analysis.Results: Mean age of participants was 54.2 years with 39.6% assessed as frail (FI >0.25). Mean time for com-pletion of FI questionnaire was 11 mins and completion rate was 100%. Participants' acceptance of the frailty as-sessment tool was influenced by ease with which it could be completed and incorporated into their outpatient ap-pointment. Participants associated frailty with increasing age and vulnerability. They often struggled to associate its relevance to themselves. Participants identified frailty measurement as a useful adjunct to clinician assessment but were apprehensive about it potentially influencing transplant eligibility.Conclusions: Mean FI among potential kidney transplant recipients is higher than the normal population although distribution reflects a younger population. FI measure-ment is feasible and acceptable to potential kidney trans-plant recipients. This study highlights the importance of educating this unique population on frailty in relation to kidney disease and how its measurement may be used in kidney transplant assessment in future.
