Abstract

In February 2013, The LOSA Collaborative implemented its 100th LOSA project. Since the first LOSA in 1996, The LOSA Collaborative has collected over 20,000 observations at 70 airline, helicopter and military organizations around the world. As a result, there have been many lessons learned about how best to implement LOSA within flight operations and support organizations wanting to be predictive with their safety management systems. The objective of this presentation is to share some of these lessons learned, along with some TEM performance findings and anecdotes. This presentation should be of benefit to any aviation organization that is currently running a LOSA program and/or designing a TEM training course.
