Abstract

Dear Sir, Platelet transfusion often requires non-ABO-compatible platelets due to high demand, specific requests (such as HLA matching or CMV negativity) and short shelf-life. ABO minor mismatched platelet transfusion can rarely cause haemolytic transfusion reactions. There are over 25 reports in the literature of acute haemolytic transfusion reactions from ABO minor mismatched platelet transfusions from group O donors (Berséus et al., 2013). There is one published report of haemolytic transfusion reactions in two recipients from a group A apheresis platelet donor where it was suggested that the high-titre anti-B was associated with probiotic use (Daniel-Johnson et al., 2009). We report a case of a severe life-threatening haemolytic transfusion reaction attributed to anti-B antibodies from an apheresis unit of group A platelets. This transfusion reaction has led to the Australian Red Cross Blood Service adopting a nationwide automated anti-A/B antibody screening protocol for all clinical donations.
